J-1 Visa Program Incident Report Guidelines

If you feel your health, safety, or welfare is endangered during your visit, here is how to file an incident report with your sponsor.

As your sponsor, our highest priority is your well-being. During your time in the United States, if you are in a situation that endangers your health, safety, or welfare, inform us as soon as possible by logging in to the portal and using the emergency button to fill out an incident report.

The following are some examples of incidents that would warrant an incident report from J-1 Exchange Visitors:

  • Dangerous and/or unsuitable living conditions (ex. crime, overcrowding, pest infestations, mold, filth, etc.)
  • Harassment, violent behavior, threats, or bullying (ex. verbal and/or physical aggressions against an exchange visitor)
  • Inappropriate training activities
  • Incidents involving the criminal justice system (ex. arrest, criminal charges, imprisonment, incarceration, detention, etc.)
  • Patterns of behavior problems (ex. substance abuse, aggressive physical contact, severe bullying, etc.)
  • Sexually related incidents or sexual abuse (incidents or allegations involving actual or alleged sexual exploitation, harassment, or any other allegations of abuse)
  • Sponsor violations (self-recognition of errors in vetting, oversight, staffing, etc.)
  • Unsuitable host conditions (exchange visitor removal from host organization for health, safety, and/or welfare reasons)
  • Work hour and wage violations involving exchange visitors (United States Department of Labor findings of violation by, or pursuit of enforcement actions against, host organizations for work hour and wage violations, or repeat work hour and/or wage-related complaints reported to sponsor by exchange visitors)
  • Lost or stolen immigration documents (ex. passport and visa or DS-2019 form)
  • Public events or natural disasters directly impacting exchange visitors’ safety (ex. bombing/shooting/stabbing/other violence, civil unrest, earthquake, fire, flood, hurricane, tornado, etc.)

The following are some examples of incidents that would warrant a report from host organizations:

  • Exchange visitor death
  • Exchange visitor missing
  • Public events or natural disasters directly impacting exchange visitors’ safety (ex. bombing/shooting/stabbing/other violence, civil unrest, earthquake, fire, flood, hurricane, tornado, etc.)
